{"id":2843,"date":"2013-04-05T09:53:54","date_gmt":"2013-04-05T13:53:54","guid":{"rendered":"http:\/\/www.l-a-k-e.org\/blog\/?p=2843"},"modified":"2013-04-05T09:55:17","modified_gmt":"2013-04-05T13:55:17","slug":"alcohol-drugs-and-broken-nuke-equipment","status":"publish","type":"post","link":"http:\/\/www.l-a-k-e.org\/blog\/2013\/04\/alcohol-drugs-and-broken-nuke-equipment.html","title":{"rendered":"Alcohol, drugs, and broken nuke equipment"},"content":{"rendered":"<p>\r\nA broken cooling water pump at Fermi 2 yesterday plus\r\n60% lost safety data today.\r\n<a href=\"http:\/\/www.nrc.gov\/info-finder\/reactor\/ferm2.html\">\r\n<img decoding=\"async\" style=\"float:right;border:none;\" src=\"http:\/\/www.nrc.gov\/info-finder\/reactor\/ferm.jpg\"><\/a>\r\nAirflow and quality problems at Kewaunee and Three Mile Island.\r\nDrugs at Saint Lucie in Florida\r\nand alcohol at Nine Mile Point 1 in New York and\r\nat Braidwood in Illinois.\r\nAnd four workers injured at Callaway in Missouri.\r\nApparently nuke employees can get terminated for off-site\r\nrecreational drug use, but not for fires or broken equipment.\r\n<\/p>\r\n<p>\r\nWe&#8217;ve already seen the\r\n<a href=\"http:\/\/www.l-a-k-e.org\/blog\/2013\/04\/fire-at-plant-vogtle.html\">\r\nevent about fire at Plant Vogtle<\/a>.\r\nHere are more events in the NRC\r\n<a href=\"http:\/\/www.nrc.gov\/reading-rm\/doc-collections\/event-status\/event\/2013\/20130404en.html\">\r\nCurrent Event Notification Report for April 4, 2013<\/a>\r\nand also today,\r\n<a href=\"http:\/\/www.nrc.gov\/reading-rm\/doc-collections\/event-status\/event\/2013\/20130405en.html\">\r\nApril 5, 2013<\/a>,\r\n<\/p><!--more-->\r\n<ul>\r\n<li>\r\na ventilation problem at\r\nThree Mile Island 1 in Pennsylvania (yes, <em>that<\/em> Three Mile Island),\r\n<li>\r\nan iodine noble gas monitor out of service at Kewaunee in Wisconsin (the\r\nnuke\r\n<a href=\"http:\/\/www.l-a-k-e.org\/blog\/2012\/12\/kewaunee-nuke-is-shutting-down-why-are-we-building-more-at-vogtle.html\">\r\nDominion already decided to shut down<\/a>),\r\n<li>\r\nat Saint Lucie in Florida,\r\n<blockquote>\r\nA licensed employee was arrested for possession of a controlled substance while off-duty. The employee&#8217;s access to the plant has been terminated. \r\n<\/blockquote>\r\n<li>\r\nat Nine Mile Point 1 in New York,\r\n<blockquote>\r\nA licensed employee supervisor had a confirmed positive for alcohol\r\nduring a random fitness-for-duty test. The employee&#8217;s unescorted access\r\nto the plant has been terminated.\r\n<\/blockquote>\r\n<li>\r\nat Braidwood in Illinois,\r\n<blockquote>\r\nDuring remodeling of a bathroom on the third floor of the Administrative\r\nBuilding which is located inside the Protective Area, workers discovered\r\ntwo very old containers of blackberry brandy after removing the ceiling\r\ntiles. This item will be entered into the licensee corrective actions\r\nprogram for follow up.\r\n<\/blockquote>\r\n<li>\r\nat Fermi 2 in Minnesota,\r\n<blockquote>\r\nAt 1053 [EDT] on April 3, 2013, during the performance of a surveillance\r\ntest on the Division 2 Emergency Equipment Cooling Water (EECW) system\r\nthe EECW system was declared inoperable due to the Division 2 EECW\r\nmakeup pump failing to start during the surveillance. The EECW system\r\ncools various safety related components, including the High Pressure\r\nCoolant Injection (HPCI) room cooler.\r\n<\/blockquote>\r\nAnd today more Fermi 2 equipment broke:\r\n<blockquote>\r\n<H3>FAILURE OF THE INTEGRATED PLANT COMPUTER SYSTEM<\/h3>\r\n<p>\r\n&#8220;At 0406 [EDT] on April 4, 2013, the Fermi 2 Integrated Plant\r\nComputer System (IPCS) failed. This resulted in a loss of\r\napproximately 60 percent of data on the Safety Parameters Display\r\nSystem (SPDS).\r\n<\/p>\r\n<p>\r\n&#8220;While IPCS and SPDS are not fully functional, the Emergency Plan\r\ncan still be implemented if a plant emergency does occur, as\r\nassessment capabilities are available under alternate means.\r\n<\/p>\r\n<\/blockquote>\r\nWhat are these &#8220;alternate means&#8221;?\r\nThe report doesn&#8217;t say.\r\n<li>\r\nToday at Callaway in Missouri,\r\n<blockquote>\r\n<p>\r\nAt 1707 CDT on 4\/2\/13 an arc flash occurred at the &#8216;B&#8217; safeguards\r\ntransformer (XMDV24) in the plant switchyard at Callaway. At the\r\ntime of the flash, ground straps were being placed on the &#8216;B&#8217;\r\nsafeguards transformer which had been removed from service for\r\nmaintenance. The event resulted in a loss of power to\r\nareas\/buildings outside the power block. There was no impact to\r\nequipment and systems in the plant.\r\n<\/p>\r\n<\/blockquote>\r\nWell that&#8217;s good.  But wait:\r\n<blockquote>\r\n<p>\r\n&#8220;Four workers were injured or affected by the flash. The extent of\r\nthe electrical-related injuries has not been determined. However,\r\nbased on reports from the scene, all of the workers were conscious\r\nand walked away from the scene. One person was transported by\r\nhelicopter and two by ambulance to a local hospital. The fourth\r\nperson experienced only a minor injury.\r\n<\/p>\r\n<p>\r\n&#8220;The hazard has been isolated and investigation of the cause is in\r\nprogress.\r\n<\/p>\r\n<\/blockquote>\r\n<\/ul>\r\n<p>\r\nSo apparently nuke employees can get terminated for off-site\r\nrecreational drug use, but not for letting fires break out,\r\nor cooling water pumps break, or losing 60% of safety data.\r\nAnd alcohol onsite can go unnoticed for a long time.\r\n<\/p>\r\n<p>\r\n -jsq\r\n<\/p>","protected":false},"excerpt":{"rendered":"A broken cooling water pump at Fermi 2 yesterday plus 60% lost safety data today.
